Lipid profile test: HDL, LDL and triglycerides explained

A lipid profile measures the fats in your blood: total cholesterol, LDL ('bad') cholesterol, HDL ('good') cholesterol, and triglycerides. Together they are one of the strongest and cheapest predictors of heart attack and stroke. This guide explains how to read each number; for symptoms and prevention, see our cholesterol and heart-risk guide. A proper 8-12 hour fast gives the most accurate triglyceride reading.

Who should test, and risk factors

  • Everyone from their 30s, as a baseline screen
  • A family history of heart disease or high cholesterol
  • Diabetes, high blood pressure, or being overweight
  • Smoking or a sedentary lifestyle
  • Already on a statin (to monitor treatment)
  • Often no symptoms at all, which is why screening matters

How to read your lipid numbers

Total cholesterol below 200 mg/dL is desirable. LDL below 100 mg/dL is optimal, lower if you are high risk. HDL is the one where higher is better, above 40 in men and 50 in women. Triglycerides below 150 mg/dL are normal. Your doctor reads these together with your overall risk, not in isolation.

LDL and the ratio matter most

LDL cholesterol is the number that builds up in arteries, so it usually drives treatment decisions. The total-to-HDL ratio is also useful. A single 'high' value is rarely the whole story. Your age, blood pressure, sugar, and family history all shape what your target should be.

Frequently asked questions

Do I need to fast for a lipid profile?

An 8-12 hour fast is usually advised, mainly for an accurate triglyceride reading. Water is allowed; a morning slot is easiest.

Which lipid number matters most?

LDL ('bad') cholesterol usually drives decisions because it builds up in arteries, but your doctor reads all four values together with your overall heart-disease risk.
