TSH and thyroid profile test: normal range and how to read it

A thyroid profile measures TSH along with T3 and T4, the hormones that set your metabolism. TSH from the pituitary gland is the most sensitive marker (it often moves before symptoms appear), which is why it is usually the first test. When to check your thyroid

How to read TSH, T3 and T4

TSH usually runs about 0.4-4.0 µIU/mL. A high TSH most often means an underactive thyroid (hypothyroidism); a low TSH suggests an overactive one (hyperthyroidism). T4 and T3 show how much hormone the gland is actually making. TSH moves in the opposite direction to the thyroid hormones, which can be confusing. Your doctor reads them together.

Free vs total, and when to add tests

Reports may show total or free (FT3, FT4) hormones; free levels reflect the active, available hormone and are often preferred. If results are borderline or you have symptoms, your doctor may add thyroid antibodies (such as TPO) to look for an autoimmune cause like Hashimoto's.
