RA factor and anti-CCP: testing for rheumatoid arthritis
Rheumatoid arthritis (RA) is an autoimmune condition that inflames the joints, classically the small joints of the hands and feet, on both sides. Two blood tests help diagnose it: RA factor (rheumatoid factor) and anti-CCP, the more specific of the two. Caught early, RA can be controlled well, which is why testing matters when joint symptoms appear. This is distinct from the wear-and-tear of osteoarthritis covered in our joint pain guide.

Symptoms that point to RA
- Morning joint stiffness lasting more than 30-60 minutes
- Pain and swelling in the small joints of both hands or feet
- Symmetrical joint involvement (same joints on both sides)
- Warmth or tenderness over the joints
- Fatigue and a low-grade fever
- A family history of rheumatoid arthritis
RA factor vs anti-CCP
RA factor is positive in many people with RA, but also in some other conditions and even healthy people, so it is not specific on its own. Anti-CCP is more specific to rheumatoid arthritis and can turn positive early, before joint damage. Together they are stronger than either alone, and your doctor reads them with inflammation markers like CRP and ESR.
What a negative result means
Some people have RA with normal RA factor and anti-CCP (this is called seronegative RA), so a negative test does not rule it out if symptoms strongly suggest it. The diagnosis is clinical, supported by these tests, imaging, and a rheumatologist's assessment. Early treatment protects the joints, so do not wait out persistent symptoms.

It supports a diagnosis of rheumatoid arthritis when joint symptoms are present, but it can also be raised in other conditions and some healthy people, so it is read with anti-CCP, CRP/ESR, and your symptoms.
Can I have rheumatoid arthritis with a negative RA factor?
Yes. Some people have seronegative RA with normal RA factor and anti-CCP. If symptoms strongly suggest RA, your doctor may still diagnose and treat it.
